1. Who we are
These Terms of Service (“Terms”) govern your use of the website and online ordering system operated by Lee Dental Laboratory (“Lee Dental Lab,” “we,” “us”), located at 8208 Harford Rd, 1st Floor, Parkville, MD 21234. By creating an account or placing an order, you agree to these Terms.
2. Who may use the service
Our service is intended for licensed dental professionals and their practices ordering dental restorations for their patients. By registering, you represent that you are authorized to place orders on behalf of your practice and that the information you provide is accurate.
3. Orders and payment
Prices are shown in U.S. dollars and are payable in full at the time you place an order. Payments are processed by our payment provider, Stripe; we do not store your full card details. Each completed order is assigned a Case ID that identifies your case throughout production. You are responsible for sending the corresponding physical impression or submitting the digital scan through your scanner system with the matching Case ID in the scan note.
4. Remake guarantee
If a restoration does not seat correctly due to a laboratory error, we will remake it at no charge when returned within ten (10) business days of delivery, subject to our review. The guarantee covers laboratory workmanship and does not cover changes to the prescription, damage after delivery, or issues arising from the impression, scan, or clinical conditions provided to us.
5. Your responsibilities
You agree to provide accurate case information, to keep your account credentials secure, and to submit only files and materials you are authorized to share. You are responsible for all activity under your account.
6. Cancellations and refunds
Because each case is custom-made, an order may be cancelled only before production begins. If you believe an order was charged in error, contact us at (410) 661-2002 and we will review the request. Approved refunds are returned to the original payment method.
7. Not medical advice
Lee Dental Lab manufactures dental restorations to the specifications you provide. We do not provide clinical or medical advice, and the treating dentist remains solely responsible for diagnosis, treatment planning, and patient care.
8. Limitation of liability
To the fullest extent permitted by law, our total liability for any claim relating to an order is limited to the amount you paid for that order, which may be satisfied through our remake guarantee. We are not liable for indirect or consequential damages.
9. Changes to these Terms
We may update these Terms from time to time. Continued use of the service after changes take effect constitutes acceptance of the revised Terms.
10. Governing law
These Terms are governed by the laws of the State of Maryland, without regard to its conflict-of-laws rules.
